Application Form for Advanced Accredited Mediator This form is to be used by mediators to apply to become an Advanced Accredited Mediator under AMDRAS. Please submit this completed form along with all supporting documentation to your Recognised Accreditation Provider (RAP). Application fees may apply as determined by your RAP.
